Episode I: The Phantom Menace

( Darth Maul loses both legs.After Qui-Gon Jinn dies from Darth Maul, Obi-Wan Kenobi continues the duel. Obi-Wan manages to turn the fight around, surprise Maul and get him close to the waist. )

Episode II: Attack of the Clones

Zam Wesell loses her right arm. After the Clawdite shapeshifter, Zam Wesell, makes an attempt in Padmé Amidala's life, Obi-Wan and Anakin Skywalker chase her through Coruscant. They find her ducking into a bar, and Obi-Wan uses his lightsaber to remove her right forearm before she can shoot him.

( Acklay loses two legs.One of the creatures in the arena during the Battle of Geonosis was the formidable acklay. It tried to defeat Obi-Wan, but he escaped its pincers by cutting off two of the acklay's front legs with his lightsaber. )

Anakin loses his right arm. When Anakin recovers from a lightning bolt from Count Dooku, he jumps back into battle and loses more than half of his right arm to Dooku's blade.

Episode III: Revenge of the Sith

Count Dooku loses both hands. At one of many turning points for Anakin, he takes revenge on Count Dooku and cuts off both of the Sith's hands with his lightsaber. (Dooku would lose his head too.)

General Grievous loses two hands. General Grievous is definitely the force to be reckoned with, given his numerous appendices. Even so, Obi-Wan cut off two of his robot hands in the fight.

Mace Windu loses his right hand. When Mace discovers that Palpatine is the Sith they have been looking for, he decides to do things himself instead of going through appropriate channels. Anakin stops Mace from killing Palpatine by removing Mace's right hand from the rest of his body.

Anakin loses his left arm and both legs. We all know how the Sith's vengeance ends, and it's not pretty. Anakin comes to hate Obi-Wan and they wage a brutal fight against Mustafar. Obi-Wan cuts off Anakin's left arm and both legs in one blow.

Episode IV: A New Hope

C-3PO loses his left arm. The Jundland Wastes must not be traveled lightly, as C-3PO has learned. He loses an arm to a Tusken Raider while trying to find R2-D2.

Ponda Baba loses his right arm. When Doctor Evazan and Ponda Baba give Luke a hard time at Mos Eisley Cantina, Obi-Wan steps in to help. He helps by cutting off Ponda Baba's right arm.

Episode V: The Empire Strikes Back

Wampa loses his right arm. Wampas are no match for power. Asking the Force to get his lightsaber, Luke cuts himself off and weakens the wampa by, as you guessed, removing her right arm.

C-3PO loses all of its limbs. Threepio is really out of luck. Or luck. This time he puts his nose on Bespin where it doesn't belong, and Ugnaughts removes his limbs and later dismantles him completely.

Luke loses his right hand. When Luke meets Darth Vader in Cloud City, he is in no way prepared for the duel. Luke manages to fight, but Vader has no problem chopping off Luke's right hand.

Episode VI: Return of the Jedi

Vader loses his right hand. Again. The next time Luke faces Vader, he'll be better prepared. He is wiser and more skillful. Before it's all over, he reverses and cuts off Vader's right hand.

Episode VII: The Force Awakens

This is the first movie where nobody loses a limb! However, if we take Alan Dean Foster's romanization as canon (and rumor has it that this was a deleted scene too) then there is a case.

Unkar Plutt loses an arm. Rey comes across Unkar Plutt, the junk dealer from whom she stole the Millennium Falcon on Jakku. Plutt pursued the hawk to Takodana and is not too happy that his valuable property was stolen from under his nose. Rey aims her newly acquired blaster at Plutt just so he can snatch it from her. Chewbacca steps in to protect Rey when Plutt whips. Chewie does what Han C-3P0 once warned about: He rips Plutt's arm out of its socket.

Others (less canonical)

( Padawan Hardin lost half a leg.At the height of the Seventh Battle of Ruusan, the Sith Lord Githany Padawan severed Hardin's leg with her light whip from the knee down. (In Star Wars: Jedi vs. Sith (Comics).) )

Balog lost a finger. Jedi Master Qui-Gon Jinn cut off Balog's finger before the treacherous Absolute could press a transmitter to end Tahl's life. (In Jedi Apprentice: The Death of Hope (novel).)

Zule Xiss lost her left arm. The dark Jedi Asajj Ventress put Cho Mok on Zule Xiss and cut off her left arm. (In Star Wars: Republic: The Battle of Jabiim (Comics).)

Nom Anor lost a limb. Leia Organa Solo used this technique at Nom Anor at the end of the Yuuzhan Vong War. (In The New Jedi Order: The Unifying Power (novel).)

UnuThul lost an arm. Luke Skywalker also severed UnuThul's arm during their duel on board the Admiral Ackbar. (In Dark Nest: The Swarm War (novel).)
